Fremont Area Medical Center’s “Taking Charge -- Diabetes Self-Management Training” program offers diabetes education and support, and is recognized by the American Diabetes Association for meeting the National Standards for Diabetes Self-Management Education Programs.
Each “Taking Charge” course consists of three, three-hour classes, which meet on three consecutive days. In August, classes are scheduled 1-4 p.m. Aug. 12-13-14.
Participants learn about symptoms, medication, blood glucose monitoring, exercise and meal planning from a Diabetes Educator and Registered Dietitian, along with other health professionals, including an internal medicine specialist, a registered pharmacist, and a physical therapist.
Reimbursement is provided by Medicare and some private insurance companies. Physician referral is required. All classes meet on the third floor of Fremont Area Medical Center’s Health Park Plaza.
